Modiwake is an oral medication designed to enhance wakefulness in individuals experiencing excessive sleepiness. Its effects are akin to those of caffeine, as both substances work to boost alertness by stimulating brain activity. Therefore, they are categorized as stimulants. Other stimulants, such as dextroamphetamine and sibutramine, also serve a distinct role in appetite suppression. While all four medications influence brain neurotransmitters—chemical messengers that facilitate communication between nerve cells—they do so in various ways.
 
Modiwake is indicated for increasing wakefulness in patients suffering from excessive sleepiness related to narcolepsy, obstructive sleep apnea/hypopnea syndrome (used as an adjunct in treating the underlying obstruction), and shift work sleep disorder. Narcolepsy is a chronic condition affecting the brain and spinal cord, primarily marked by recurring, uncontrollable urges to sleep. Shift work sleep disorder disrupts the normal sleep cycle, often due to night shift work.
 
Typically, Modiwake is administered at doses of 200 or 400 mg daily; however, the 400 mg dosage has not demonstrated greater efficacy than the 200 mg dosage. Modiwake may be ingested with or without food.
 
Modiwake can either enhance or inhibit the activity of liver enzymes responsible for metabolizing (clearing) other medications. This could lead to reduced effectiveness of some drugs due to decreased levels, or increased toxicity of others due to heightened levels. Therefore, careful monitoring is essential when Modiwake is used alongside these medications.
 
Medications that may experience diminished effectiveness when taken with modafinil include cyclosporine, theophylline, and hormonal contraceptives like progestin-only or those containing estrogen and progesterone. It is advisable to consider alternative hormonal contraceptives or a non-pharmaceutical form of contraception while undergoing Modiwake treatment, extending at least one month after the cessation of therapy (since Modafinil’s impact on contraceptive hormones may persist for several weeks). The interaction of Modiwake with hormonal contraceptives occurs with oral, patch, or implant methods, and it remains uncertain whether interactions exist between Modafinil and alcohol.
 
The most frequently reported side effects of Modiwake during clinical studies include headache, upper respiratory tract infections, nausea, nervousness, anxiety, and insomnia, each occurring in over 5% of patients.
